New Patients

Your first visit to Dermatology Consultants Midwest establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. To understand what to expect for your first visit to our practice, please read through this page.

Please complete the medical history and new patient paperwork forms online. Information regarding access to these online forms will be given at the time you make an appointment.

At Dermatology Consultants Midwest, P.A. we strongly feel that every patient should be evaluated by a board-certified  dermatologist. So regardless if you are a new patient or an established patient returning for a follow-up examination, you will see the physician every time.

What To Expect

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It will also help alleviate any unnecessary anxiety you may be feeling prior to your first appointment. Educate yourself on your symptoms by reviewing the content on this Web site. Also, take some time to review our staff page and familiarize yourself with the doctors. We look forward to your first visit.

New Patient Instructions

Our office has a patient portal which will allow you to complete the required patient paperwork prior to your first appointment.  Please make sure to read the full instructions list below to help you create an account and complete your new patient paperwork.  

Step 1: SADIO is the NEW Patient Portal.  You should receive an email to verify your email address.  If you do not receive this email, please contact our office to verify the email address we have on file.

Step 2:  Click on the verification link in your email.

Step 3:  Create a SADIO account with a username and password.       

Step 4:  Enter your Medical History, including your prescriptions.  If you do not know how to spell them, type the first few letters and suggestions will appear in the dropdown.  

Step 5:  Choose your pharmacy.  You can search by name, city or zip code.  In order to select the pharmacy, tap or click on the green star.   You can choose multiple pharmacies.

Step 6:  Sign consent forms.  For SADIO, these cannot be signed on a phone, but can be accessed on a PC, Mac or iPad.  

Step 7:  You will ALSO receive a text message reminder prior to your appointment.  Once you confirm, you will be able to sign any unsigned consents on your phone, upload your driver's license and insurance cards securely to our portal and complete your medical history.
