At Dermatology Consultants Midwest, P.A. we strongly feel that every patient should be evaluated by a board-certified dermatologist. So regardless if you are a new patient or an established patient returning for a follow-up examination, you will see the physician every time.

New Patient Instructions
Our office has a patient portal which will allow you to complete the required new patient paperwork prior to your first appointment. Please make sure to read the full instructions list below to help you create an account and complete your new patient paperwork.
Step 1: To access the portal click the following link: https://mypatientvisit.com/#/login?practiceID=DZLTRT or copy and paste the link into a new browser
Step 2: From the home page, under Don’t have an Account, click the Create an account link on the right side of the screen.
Step 3: Fill in all Sign-In information in the provided fields creating your own username and unique password for the account. You will also need a security code provided to you by our office which is listed above. NOTE: The first and last name, date of birth, and zip code you enter must match the information on file with our practice. PLEASE MAKE SURE TO COMPLETE YOUR MEDICAL HISTORY IN THE FORMS TAB. If you are having difficulty, please call us at 913-469-0110.
Step 4: Read the full Terms and Conditions and click the acceptance checkbox. Click Register.
Step 5: Select 3 Security Questions from the dropdown menus, input appropriate answers and submit.
Step 6: Click Connect.
Step 7: Patient Dashboard will appear. From here, you can access tools and information via your secure patient portal.
Submit your health history form by following the navigation bar at the top of the screen to my Info, Patient Information and my Documents Forms. Please be sure to CLICK SAVE & SUBMIT after completing the forms. SKIP THE INSURANCE FIELD AS THAT WILL BE ENTERED AT YOUR APPOINTMENT.
