Policies and issues regarding Check-In, Check-Out and Scheduled Appointments
Appointments
* Patients are seen by appointment only. We cannot accommodate Walk-Ins. Please call our office to set up your visit.
* Please call to cancel as soon as you are aware that you cannot keep your appointment. Except in the case of emergencies, we request 24 hours notice of your intent to cancel.
Check-In
* This process involves confirming your demographic, insurance and medical history information. Your appointment time is the time you should be called to the exam room so we need you to arrive prior to this to complete check-in. Your current and effective insurance card must be presented at each appointment.
Below is a guide to the times we require you to arrive for your appointment:
New Patient Visit (includes Established patients not seen in last 3 years) and
Established Patient Not seen since prior to July 1, 2011:
15 minutes
Subsequent visits once established in Patient Portal: 5 min. if no changes and 10 min. if changes in insurance and/or medical history.
BE AWARE- There are times when a doctor is completely booked and may not be able to accommodate patients who do not arrive in time to allow completion of any needed check-in procedures.
Check-Out
* ALL co-payments and applicable deductible balances are due at the time of service. Any payment related to a procedure or treatment deemed as “not medically necessary” or cosmetic is also due at the time of service.
