Visits are by appointment only.
Call 913-469-0110 to set up your appointment. Inform us if you have seen any of our physicians before- even if at another location. Please have your insurance information available (specific plan name) so that we can confirm our participation with that plan. If you are a new patient or a returning patient not set up with a patient portal a security code will be given to you that will allow you to set up a log-in for your Patient Portal. Read the instructions provided completely prior to sitting down to fill out the information on line.
If you suspect that you were last seen in the practice more than 3 years ago and/or if you may have had a name change since last to the office, please let us know.
Your appointment time is your time to see the doctor. We strive to stay on time as much as possible- use the Patient Portal to streamline your check-in and refer to the Front Office page for guidelines on arrival times. Be sure to bring insurance confirmation (card) and photo ID. The Patient Portal is now on line and we will start have you use this to enter your personal, insurance and medical history information prior to your initial (or next) appointment. Note: It is not required that you fill in a Social Security Number on the on-line form, however we will need to get this information once you check in at the office as it is required for insurance reimbursement.
We request a 24 hour notification on any cancellations or changes to your appointment time.
